    Specially designed to provide short-term recovery care you need when facing multiple medical problems/illnesses after a hospital stay.
  • Pain Management
    Patients who may benefit from these services include individuals who are suffering from acute or chronic pain. Individuals who recently had surgery or experienced trauma from a fall. Pain from osteoarthritis, poor blood circulation or Diabetic Neuropathy.
  • Physical Therapy
    Instruct patients in techniques to improve their ability to walk, climb stairs and increase physical stamina. They assess the need for specialized braces or equipment that may be required to improve self-sufficiency and safety, and work on pain management skills without the dependence on opioids.
  • Cardiac & Stroke Recovery Program
    When you need time to recover from a cardiac procedure or conditions such as heart failure, high blood pressure, or stroke recovery, our facility can provide the specialized cardiac & stroke recovery services you need to continue on the road to home.
  • Memory Care or Alzheimer’s care
    Our Memory care or Alzheimer’s care unit is specially designed to meet the needs of adults with Alzheimer’s and other types of dementia. Our employees undergo special training to help with communication and the management of dementia symptoms like Sundowner’s syndrome, wandering, or agitation.
  • Adult Day Care
    This program allows you as the primary caretaker to go shopping, have lunch with friends, or continue to work full-time while we care for your loved one. Adult day hours are from 6:00 a.m. – 6:00 p.m. 7-days a week.
  • Occupational Therapy
    Therapists take individuals through a variety of daily living activities such as bathing, dressing and preparing meals. This may include using a bathtub seat, grab bars or long-handled sponge for bathing or to use a reacher to obtain objects from shelves, and cognitive functioning, such as improving short-term memory and reaction times.
  • Respiratory Therapy
    Focuses on pulmonary illness, such as respiratory failure, COPD, congestive heart failure, or pneumonia, respiratory therapy often avoids your return to an acute care hospital. In fact, avoiding hospital readmission is one of the main goals of providing these and other high-quality rehabilitation services.
  • Wound Management
    Patients who may benefit from these services include anyone with a persistent wound. Chronic and non-healing wounds can result from a number of factors such as diabetes, poor circulation, trauma, vascular disease and immobility.
  • 2-Tier Rehab Program
    Designed for patients that can tolerate a higher level of therapy. This program utilizes 6-7 day a week skilled therapy plus a 5 day a week Restorative Nursing Program that is based on skilled therapy goals, thus allowing patients to return home usually in less than 20 days.
  • Speech Therapy
    Focuses on speech, language, voice, cognition and swallowing problems specializing in stroke recovery.
  • Hospice Services, "End of Life" care
    Our complete list of the hospice companies that NLNR has a contract with includes: Heart to Heart Hospice of Fort Wayne, Promedica Hospice, Parkview Hospice, Dekalb Hospice, and Lutheran Hospice.
  • Respite Care
    Respite care is an option for those who need respite care services for your loved one on a short term basis (less than 30 days). This allows home caregivers the ability to travel, recuperate from an illness, or pursue other interests.
  • Parkinson’s Therapy
    Therapist including Physical and Speech therapy works together to improve balance, trunk rotation and walking, as well as improve lung capacity, voice volume and speech functions while developing a fitness program to continue at hone.
  • Post-Discharge Home Visit Wellness Program
    Courtesy nursing service: Post-Discharge Home Visit Wellness Program 4 weekly home visits to patients who have been discharged home from NLNR. The purpose of this program is to keep patients safe in their homes and prevent any unnecessary emergency room or hospital visits. Wellness checks can include: check medications, complete a set of vitals, review of discharge folder & plan and ensure patient's general well being.
  • Long-term Care
    Long term care includes a variety of services for individuals who cannot care for themselves for extended lengths of time. When you or your aging loved one can no longer be cared for in the home, long term care offers around the clock care by a specially trained team of healthcare professionals.
  • Semi-Independent Living
    Semi-Independent Care is for healthy seniors that only require minimal oversight & assistance for daily routines. To qualify for Semi-independent living, the senior must be: Ambulatory, able to move under own power Independent or Supervision for showers/bathing Independent for personal hygiene Independent with ability to eat. Semi-independent living advantage is the senior will still receive the following medical services & oversight: Medication Administration & Management Medical oversight for chronic health conditions. Physician on-site visits every 60 days and with change of condition. Including 24-hour on call physician services. Diabetic Management including Blood Sugar Monitoring, Insulin Administration On-site Lab & Radiology Services 24-hour Nursing Support On-site Podiatry, Audiology, Dental, & Ophthalmology Services Therapy Services Pharmacy Services On-site Dietitian Services providing specialized meal plans for Diabetes, Cardiac diets including Low Sodium Diets, Gluten Free Diets. Social Services Featured Amenities: Semi-Independent Care On-site Beauty/Barber Services with monthly Cut & Set, or Men’s Cut at no additional charge. Free Satellite Services Free Wi-Fi Personal Items Supplied at no additional cost including.
